(Provided by Trained Companions under RN Program Oversight — for clients who are alert, independent, and do not require medication management or close monitoring.)

Accompaniment to community or recreational activities
Assistance with errands, shopping, and appointment scheduling
Daily check-in calls or wellness reminders

This service level does not include clients with dementia, Alzheimer’s, or cognitive impairment, and you can't be incontinent or require medication administration or direct clinical oversight.

(Provided by Certified Medication Technicians (CMTs) under Registered Nurse Delegation — for clients requiring closer monitoring, medication setup, or complex personal care.)
Assistance with bathing, grooming, dressing, and hygiene
Toileting and incontinence care with attention to infection prevention
Mobility and transfer assistance with fall prevention techniques
Meal preparation and feeding support with dietary monitoring (e.g., diabetic, cardiac)
Light housekeeping and laundry within the care area for safety and cleanliness
Medication reminders, setup, and administration (per RN delegation)
Monitoring for changes in health status — fatigue, swelling, confusion, skin changes, etc.
Documentation of care and updates to the supervising RN
Escort to medical appointments with handoff notes to/from providers
Emotional support, reassurance, and dignity-centered care
Collaboration with RN for medication reconciliation and care plan updates
Support for clients with mild cognitive impairment, early dementia, or chronic conditions
Skilled Nursing Oversight and Add-Ons

Initial nurse assessment and care plan development
